Bij het verzoekschrift voeg je een geneeskundige verklaring van een arts, waarin de gezondheidstoestand wordt aangetoond. Deze verklaring mag niet ouder zijn dan 15 dagen. Je mag daarnaast extra bijlagen toevoegen, zoals schoolverslagen om de gezondheidstoestand te onderbouwen. Je kunt ook al een suggestie doen wie je als bewindvoerder wilt aanstellen. De vrederechter kan afwijken van deze voorkeur, maar moet dan wel uitleggen waarom.
Kosten voor het indienen van een verzoekschrift bewindvoering zijn 24 euro. Je kunt een bewind aanvragen vanaf dat de persoon in kwestie 17 jaar is. Het bewind zal dan ingaan op de 18e verjaardag. Zo vermijd je een 'bewind loze' periode na de 18e verjaardag.
