Ortho-agogisch centrum Sint Jan de Deo in Kortemark is een door het VAPH vergunde zorgaanbieder, een voorziening voor personen met een verstandelijke handicap. Er zijn 7 leefgroepen voor zo'n 70 bewoners. In 2017 zijn er 5 studio's gerealiseerd op het terrein van de voorziening waar volwassenen zelfstandig kunnen wonen.
Wie kan er in de studio's wonen?
De studio's zijn beschikbaar voor personen met een (vermoeden van een) handicap. De bedoeling is dat zij zo maximaal mogelijk zelfstandig wonen. Er wordt eerst gekeken naar wat de persoon zelf kan en vervolgens wordt bekeken welke hulp er nog extra nodig is, en wie deze mee verder kan ondersteunen.
Hoe werkt het?
De kandidaat huurder meldt zich aan met een ondersteuningsplan. Dit plan is het uitgangspunt om de ondersteuning op maat verder uit te bouwen om zo een maximale levenskwaliteit voor de huurder te bekomen. In dit ondersteuningsplan staat duidelijk omschreven hoe de persoon zijn/haar leven verder zal uitbouwen, wat zijn/haar mogelijkheden zijn, waar er ondersteuning nodig is en wie deze ondersteuning zal bieden. Kandidaten die nog niet beschikken over een ondersteuningsplan kunnen zich ook melden. Hun vraag wordt eerst met de Sociale Dienst besproken.
Welke hulp/ondersteuning kan geboden worden?
Eerst wordt bekeken welke steun het eigen netwerk kan en wil bieden. Daarnaast is er thuishulp, thuisverpleging en poetshulp mogelijk. In geval van specifieke ondersteuning kan beroep worden gedaan op zorgvoorzieningen, dit kan Sint Jan de Deo zelf zijn, maar kunnen ook andere voorzieningen in de buurt zijn. Vanuit Sint Jan de Deo wordt tenslotte algemene begeleiding geboden. Dit is bijvoorbeeld een regelmatig bezoekje bij de bewoner, een luisterend oor bieden, helpen bij administratie en stimuleren van beweging, hulp bij bereiden van gezonde voeding,....
Zijn er gezamenlijke activiteiten?
Ja, er worden activiteiten georganiseerd waar alle studiobewoners aan kunnen deelnemen. De studiobewoners zijn vrij om hier al dan niet aan deel te nemen. Bijvoorbeeld samen koken, kaartjes schrijven voor bewoners van een woon-zorgcentrum of sinterklaas vieren. Het is ook mogelijk om deel te nemen aan de activiteiten die de voorziening organiseert.
Wat zijn de troeven?
De bewoners wonen zelfstandig en kunnen hun eigen ding doen, maar wonen tegelijkertijd in een warme leefomgeving met andere bewoners en cliënten om hen heen. Er zijn dag en nacht medewerkers van Sint Jan de Deo aanwezig. Ze kunnen deelnemen aan de grotere algemene activiteiten van de voorziening (los van de dagbesteding) en er warme maaltijden bestellen. En er is een grote tuin waarvan ze kunnen genieten.
Hoeveel studio's zijn er?
Er zijn vijf studio's; vier gewone studio's van 41m² en één iets grotere studio van 47m². Alle studio's hebben een ruime woonkamer, open keuken, badkamer, berging en deels afgescheiden slaapkamer. De grootste studio heeft wel een aparte slaapkamer.
Wat is het kostenplaatje?
Huurprijs: € 450,- per maand (elektriciteit, water, verwarming, internet, tv-aansluiting en brandverzekering inbegrepen). Voor de algemene begeleiding worden 8 zorggebonden punten (met Persoons Volgend Budget of Rechtstreeks Toegankelijke Hulp) gerekend per jaar.
Wat is de visie?
Het is belangrijk dat mensen zich thuis voelen. Respect, open communicatie en samenwerking zijn van grote waarde. Respect voor de privacy van de bewoner, want het is tenslotte hun huis. Daarnaast is het van belang dat de bewoner een volwaardig lid van de samenleving is en één of andere vorm van dagbesteding heeft. Dit kan bijvoorbeeld in een maatwerkbedrijf zijn of vrijwilligerswerk.
